Fill out the entry form. You will be asked to provide your name, date of birth, height, measurements, hair color, eye color, hometown, occupation, hobbies, favorite quote or comment, e-mail address and telephone number.

Modeling Agreements
Quickk Magazine/Quickkmag.com will agree to promote each model featured on our site in a professional manner. Models will submit 5 - 10 of their favorite photos to be featured on the site. This agreement allows Quickk Magazine/Quickkmag.com rights to use photos. Quickk Magazine/Quickkmag.com and its staff will choose photos featured on our web site. All imagery and model profile documentation will be submitted for approval to model before any related material is posted to our site.

Terms
Models will not be paid for being featured on our site. We do not allow any nudity, pornography, multi-level marketing schemes, on Models sites. Models must be 18 years of age or older. Proof of age must be submitted. All profile data must be accurate before being posted to our site. We accept no responsibility for any interaction or contact by others. After being featured on our site, models must commit to take 3-5 photos, one in public wearing one of our promotional t-shirts. Models must commit to submitting photos to our offices within 2 months after being featured on our site. Quickk Magazine/Quickkmag.com will supply T-shirts. During the development and on the completion of the project, all project files will be transmitted to Quickk Magazine/Quickkmag.com via electronic medium. If any project files are requested or delivered other than as specified above, model understands Quickk Magazine/Quickkmag.com will not be required to cover any expenses. Indemnification Model agrees to indemnify and hold Quickk Magazine/Quickkmag.com, its parents, subsidiaries, affiliates, officers and employees, harmless from any claim or demand, including reasonable attorneys' fees, made by any third party due to or arising out of Model promotion. No responsibility will be assumed by Quickk Magazine/Quickkmag.com. for late or misdirected entries, entries that are lost due to disconnection, incomplete, or unintelligible information provided by entrants, the U.S. Postal Service or for telephone system and/or equipment failure, or data loss.

All entries become property of Quickk Magazine/Quickkmag.com. and will not be returned.

All employees of Quickk Magazine/Quickkmag.com. (and their family members) are prohibited from entering. Void where prohibited by law.

Models must grants Quickk Magazine/Quickkmag.com. the right to use his/her name and likeness in advertising and promotion without further compensation or permission, except where prohibited by law. Models are required to sign and return a statement of eligibility and publicity/liability release which must be returned within forty-eight (48) hours of being featured on or site.

By accepting these terms model agree that Quickk Magazine/Quickkmag.com. will not be held responsible for any loss of property, accidents, injuries, or death that may occur.
